Cancer Perforation of the Stomach and Omeztoplasty






Abstract
A large perforation of the stomach cancer surrounded by neoplastic induration in a debilitated and undernourished patient with extensive peritoneall soiling presents a difficult, problem with its closure. Since sutures cut through the rigid, ulcerated and thin canoe induration, a simple closure of such perforation is not comfortable as benign, ulcer-perforation. Peforation of stomach cancer has occurred in 3.2% of 278 operative eases of stomach cancer during 6 years period from August 1968 to June 1974.When the patient is not subjected to primary gastric resection, a large perforation of the stomach cancer has better managed by means of an omentoplasty, not an omentopexy. Among 9 cases of stomach cancer perforation, reviewed here, omentoplasty was done for five instances of relatively large perforation, and simple closure by means of omentopexy was adequate for four of small perforations. Omentoplasty seems to be an acceptable technique to close a large cancer perforation of the stomach.
